CAQ in Palliative Medicine and Hospice

    Hello Everyone!

    I met a few of you last year at the 2022 Conference in Indianapolis. 
    I am registered for the Spring PA certification class at CSU Shiley Haynes that starts beginning of March - YEAH!!!
    And I just registered on the NCCPA website for the CAQ Exam.

    How do you suggest I go about logging the 2000 hours and Procedures/Patient Case requirement?  

    I have many years of hospitalist work during COVID - Can I just add up all my shifts and make that a log?  Or do I need to come up with individual patient cases?  

    And now that I'm on the outpatient end, I should just start logging dates, patient names and then procedures to include END of life discussions, POLST forms, etc??

    And then where do I get the form to have a supervising physician sign off my hours?  I'm looking on the NCCPA website but cannot find it?

    Also, I have been asked by University of Washington Physician Assistant program to give the presentation on Palliative/Hospice care this May! Woot woot!
